Don't forget to return your prescription form to use if you have not already. Forms are due in our office by October 31st in order to give our staff enough time to complete the applications.
If you have a doctor's appointment scheduled after October 31st and are unable to see your doctor sooner, please call our office to let us know. The last date to submit change for the Part D plans is December 7th and we want to make sure we have plenty of time to process any necessary changes.
Any plan changes made will not go into effect until January 1, 2019.
